Shelter

They offer shelter, counselling, life skill programs, access to a doctor and psychiatrist, harm reduction and employment training.


For Adults and Families

Central intake number 416-338-4766 (they are open 24 hours a day)
First question:

Is the person on Ontario Works (welfare/social assistance)?  Do they have children under 18 years of age?

If so, they need to call their Ontario Works Case Worker (before 4:30 pm when they close) and say: 

1- homeless
2- evicted and on the street
3- requires "emergency bridging funds" (MUST SAY THIS AS THEY CAN SEARCH THEIR DATABASE FOR THESE KEY WORDS AND WILL HAVE FUNDING AVAILABLE) The 
Emergency bridging funds pay for a hotel until the individual and children get family shelter (I was told that this is a long process so the person and children need to get temporary shelter)

Step two is to call the central intake number 416-338-4766 (they are open 24 hours a day) 


If the person is not on Ontario Works then they call the central shelter intake line 416-338-4766 and go that route.  They may be able to look into it for the person to see if they can get Ontario Works.

Children without opportunity to attend school regularly before arriving in Canada can apply to LEAP (Literacy Enrichment Academic Program) through the student’s home school. LEAP includes English language, literacy, and mathematics skills.

Transportation

Rides to and/or from medical appointments, the A Day Away Club, and the Active Living Centre.

Monday to Friday 8:30-4:30.

Eligibility: 

  • Must be members of and registered with their transportation department
  • Must live in catchment area. Exceptions made for the following destinations: Valleyview Residence, Baycrest Centre for Geriatric Care, Sunnybrook Health Centre, Toronto Rehabilitation Centre, and CNIB.
  • Are unable to use public transit because of physical or cognitive limitations
  • Users are not eligible for Wheeltrans
  • Vehicles are NOT WHEELCHAIR ACCESSIBLE: users must be able to access vehicle independently or with minimum assistance


Active Living Centre

For adults 55+

Yearly membership of $55 for first year (in addition to a mandatory $50 one-time fitness assessment) and $50 membership for subsequent years.
Access to fitness centre, all virtual and on-site programs, art room, café, and general interest classes. Reduced rates on trips, workshops, and events. 


Adult Day Program

Monday to Friday 9:30 am – 3:00
For individuals who are frail or have cognitive challenges. Engages clients in activities and social time that encourages development of greater self esteem and independence, and allows caregivers to rest and recharge. Hot drinks are offered at the beginning of the day, and there is a nutritious meal for lunch.

Full program day is $28.50, with transportation it is $41.00.


Caregiver Support

Meets 3rd Tuesday of the month 4-5:30 pm
Personal Care Workers are available for the senior not at the support group.


Home Care

Paid services include personal care (bathing, dressing, toileting and hair washing), light housekeeping, light meal preparation, and companionship.

Volunteers provide friendly visits and telephone calls to homebound seniors to help them create a social connection and feel less isolated. During visits, activities may include short walks, reading, discussing current events or playing cards. Friendly Visiting and Telephone Reassurance Calls are free and available to seniors between 9:00 am to 5:00 pm. who reside within our service area.

Assisted door-to-door transportation services to seniors 55+ and adults with disabilities who are not eligible for Wheel-Trans. 

Transportation services provided Monday through Friday, 8:30 am - 4:30 pm.

Rides can be booked up to 30 days in advance. Same-day requests subject to availability. Cancellation fee (equivalent of a one-way fare) may be applied to rides cancelled after noon the previous business day. 

Catchment area (most of Toronto): Boundaries are within HWY 427 in the west, Steeles Ave. in the north, Victoria Park in the east, and southbound to the lake.

0 to 5 km = $6.60

5.1 to 10 km = $10.50

10.1 to 15 km = $15.25

15.1 to 20 km = $18.75
